Patient Membership
This patient membership agreement (the “agreement”) specifies the terms and conditions under which you the undersigned patient (“Patient”) may participate in the program (“Program”) offered by “My Doctor & Me” and your physician. This agreement will become effective either on the date your physician commences the program or the date of your signature on this Agreement, whichever is later (the “Effective Date”).

“All Inclusive Care and Prevention package”
Includes:
- All primary care and outpatient cardiovascular needs.
- Annual history and physical examination performed and one comprehensive “mid-year’ scheduled assessment.
- All scheduled follow up visits and up to 4 requested “sick visits” per year.
- 24/ 7 direct access to your Physician and care team.
- Assigned personal Medical Assistant to facilitate implementation and administration of your care.
- Full annual baseline general wellness and cardiovascular status assessment.
- All listed diagnostic tests ordered and performed by your doctor at no additional charge.
- Assistance and guidance for all aspects of your and your families care plan needs. The goal of which is to assure all aspects of your care; primary & cardiovascular outpatient care, chronic specialist care, hospital and emergency care are economically covered with the least risk to you.

“Total Care and Prevention”
Includes:
- All primary care and outpatient cardiovascular care needs.
- Annual history and physical with full health and cardiovascular risk assessment.
- 24/7 direct access to your physician and care team
- Full cardiovascular risk assessment with all indicated diagnostic services ordered by your healthcare team included.
- Appointment times booked for a minimum of 1 hour (assures minimal to no waiting).
- 40% discount on nonincluded services such as special labs or treatments.
- All follow up visits and up to 4 sick visits per year.
- Full prevention lab package
- 1 visit to outline and agree to a health management and prevention plan, designed to be executed in the years to come to have the greatest chance to live a long life with the highest quality.
- My Doctor & Me, your de facto healthcare advocate and partner by negotiating cash prices with other healthcare providers and advising on any healthcare related issues. Scheduling of outside consults, referrals, and imaging. Referrals that require “cash payments” will be negotiated on your behalf before visit is confirmed. In cases where My Doctor & Me pays on your behalf you will pay negotiated amount directly to My Doctor & Me.
- Work with you to make medications affordable through manufacture rebates and assistive programs.
- Provide education and guidance when signing up for health insurance programs, especially Medicare.

Includes:
- All primary care and outpatient cardiovascular care needs.
- Full cardiovascular risk assessment with all non-Medicare covered services in our care package included. Medicare covered services will be billed by the provider and My Doctor & Me will pay your non-covered patient responsibility related to the services provided by your doctor as determined by Medicare.
- 24/7 direct access to physician medical team.
- 40% discount on non-covered cash services
- Annual History and Physical exam with a 6-month follow-up visit.
